I hereby apply for membership of the Ballina RSL Club Limited. I am over the age of 18 and I have read and understand the Club?s privacy statement on the reverse of this form. I certify I understand and will abide by Ballina RSL Club?s Constitution, By-laws (available at reception) and all other rules and policies of the Club. I understand that club membership commences on 1st July each year and expires on 30th June. Membership application and renewal paid for during this period will still only be valid until 30th June at which time membership renewal will again fall due. On completion of this form and once payment is processed, you will be a provisional member of the Club pending approval by the Board. I consent to receive occasional updates about my membership & club promotions. The Ballina RSL Club Limited will adhere to the Australian Privacy Principles (APPs) aimed at ensuring the information the Club holds about people is managed responsibly. Adoption of APPs also give people some control over the way information about them is handled by the Club.
We will only record your e-mail address if you provide that information on your membership application form. The Club will not disclose this information to outside organisations.
